FAILURE TO RETURN THE COMPLETED C.O.L FORM BY THE STATED DEADLINE DATE WILL RESULT IN THE SUSPENSION OF BENEFIT PAYMENTS UNTIL SUCH TIME THAT THE C.O.L IS DULY RETURNED.
